چکیده
A 36-year-old female was referred with renal impairment, creatinine clearance 0.4 mL/s and proteinuria 4.7 g/day. Physical examination was unremarkable. Biochemistry showed normochromic normocytic anaemia, slight hypoalbuminaemia and hypercalcaemia. Anti-neutrophil cytoplasmic antibodies, anti-nuclear antibodies, anti-glomerular basement membrane, anti-phospholipid antibodies, and viral screening were negative. IgG-kappa M-protein was found in plasma (4.8 g/L) and urine (<0.02 g/L), while plasma immunoglobulins and liver enzymes were normal. Bone marrow, skeleton scintigraphy and MR scan were normal. An ultrasound showed normal-sized kidneys. Kidney biopsy presented focal interstitial nephritis, light grade

Combined immunoglobulin G kappa nephropathy: monoclonal immunoglobulin deposition disease and proximal tubulopathy: monoclonal gammopathy of renal significance or smoldering multiple myeloma? Case report and review of literature
Multiple myeloma (MM) is consistently preceded by precursor states of monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM). These represent a continuum of progression of the tumor burden from the absence of symptoms or signs of end-organ damage towards full-blown symptomatic disease. MGUS, by definition presenting by monoclonal gammopathy without end o...
متن کاملMonoclonal Gammopathy of Undetermined Significance with Metastatic Carcinoma – A Rare Presentation
The presence of a low level of M protein in the peripheral blood <3gm/dl which is below the required cut off for the diagnosis of plasma cell myeloma is known as monoclonal gammopathy of undetermined significance (MGUS). Elevated levels of serum M protein and detection of M band on serum protein electrophoresis (SPE) can occur in chronic diseases not related to B cell disorders and not always d...
متن کاملProgression of Monoclonal Gammopathy with Undetermined Significance to Multiple Myeloma Diagnosed by Kidney Biopsy: A Case Report
Monoclonal gammopathy with undetermined significance (MGUS) carries a risk of progression to multiple myeloma, and progression is usually diagnosed with changes in M-protein or bone marrow biopsy. We report a case of 62-year-old female patient showing MGUS progression to multiple myeloma without significant changes in M-protein but diagnosed by kidney biopsy. During the follow-ups, azotemia and...
متن کاملAcquired Fanconi syndrome is an indolent disorder in the absence of overt multiple myeloma.
Adult-acquired Fanconi syndrome (FS) is a rare complication of monoclonal gammopathy. We retrospectively reviewed 32 patients diagnosed with adult-acquired FS between April 1968 and June 2002 at Mayo Clinic (Rochester, MN). At diagnosis, most patients had monoclonal gammopathy of undetermined significance (MGUS) or smoldering multiple myeloma (SMM), with a median creatinine level of 176.8 micro...
متن کاملMonoclonal gammopathy of renal significance: when MGUS is no longer undetermined or insignificant.
Multiple myeloma is the most frequent monoclonal gammopathy to involve the kidney; however, a growing number of kidney diseases associated with other monoclonal gammopathies are being recognized. Although many histopathologic patterns exist, they are all distinguished by the monoclonal immunoglobulin (or component) deposits. The hematologic disorder in these patients is more consistent with mon...
